WE DO NOT NOT FILL OUT DENTAL INSURANCE CLAIM FORMS
FOR THE MARKET PLACE/OBAMACARE- WE DO NOT PARTICIPATE

Midwest Holistic Dentistry strives to offer exceptional holistic dental care., outstanding customer service, and an unparalleled patient experience. Dr. Johnson does do all of the dentistry here in our office except for dental cleanings. We do not participate in ANY dental networks which allows our staff to give each patient the time to ask questions, receive only high quality dental work and materials, and not be rushed in and out of the office. In most dental office’s that do accept dental insurance payments the dentist’s will hire a certified dental assistant who can place fillings after the dentist has removed the decay or old filling. This allows for that dental practice to operate on volumes of patients per day. Our office does NOT DOUBLE BOOK patients. We see one at a time. Dr. Johnson is a perfectionist and wants to make sure your dental work is done to the highest standard of care. You will leave with an exceptionally high quality of dentistry that will last for years to come. We take pride in our porcelain crowns and dental composite fillings looking natural and feeling natural to the tongue and lips.

Most of our patients do not have dental insurance, but for the patients that do, we are more than happy to fill the dental forms out completely and we will submit the forms for you. Most of our patients are typically reimbursed within a two week period. For the patient who does have dental insurance., it is your responsibility to give all necessary information at your first appointment for the claim to be submitted correctly, to follow up on your dental claims with the Insurance carrier and to check with the insurance carrier about coverage. Midwest Holistic Dentistry cannot guarantee any reimbursement for any procedure. In most cases, most dental insurance carriers reimburse for substandard dental treatments such as mercury filling and non-precious crowns and to pay the least amount for services and to minimize claims.
